Care Management Integration Director



This position will be a crucial member of the implementation leadership team and serve as an expert in the build out of initial market Care Management programs and services as well as serve as a liaison to local the market leadership. Additionally, this role will provide Care Management subject matter expertise in the design and deployment of ongoing product implementations. This role will be part of the Care Management Leadership team but day to day work will be in partnership with the activities and goals of the implementation team.

Essential Duties and Responsibilities

  • Serves as Care Management Subject Matter Expert for Implementation team, appropriately translating Care Management  needs and considerations to implementation activities
  • Identifies impacted parties, business partners, and resources required to improve operational practices
  • Works closely with Implementation team to scope new market and new product implementations; understands impact on Care Management
  • Partners with market teams to adapt implementations to market dynamics
  • Collaborates closely with the regional Care Management teams to understand roles and responsibilities of Care Management programs and services
  • Develops and designs processes and systems that support business needs, leveraging industry best practices
  • Assists with new market launches, particularly in set up of daily operations of a large clinical enterprise caring for complex patients
  • Works with Care Management team to develop compliance, quality, and operational standards and takes responsibility for market implementation
  • Executes handoffs of Care Management  implementations to local market teams and regional Care Management leadership
  • Highly organized and able to build structure for others; passion for operational excellence and continuous improvement to enhance efficiency and effectiveness of front-line teams and patient care
  • Leads initiatives to achieve operational excellence across clinical and non-clinical areas




  • BA/BS degree; Master’s preferred (MBA/MPH/MPP/MHA)
  • 8-10 year experience in Care Management program development and oversight within a managed care environment
  • or any combination of education and experience which would provide an equivalent background to support
