This position is responsible for ensuring that appropriate members receive their prenatal and/or postpartum appointments to meet quality goals. Primary
duties may include, but are not limited to:
  • Coordinates follow-up care needs for members by scheduling appointments or referring members to programs/resources
  • Assesses member compliance with medical treatment plans via telephone
  • Identifies barriers to plan compliance and coordinates resolutions
  • Identifies opportunities that impact quality goals and recommends process improvements
  • Coordinates identification of and referral to local, state or federally funded programs
  • Coaches members on ways to reduce health risks
  • Prepares reports to document case and compliance updates
  • Establishes and maintains relationships with agencies identified in appropriate contract

Qualifications

Requirements for level I:
  • High school diploma, GED, or equivalent or a minimum; or 1 year of related experience; or any combination of education and experience, which would provide an equivalent background.
  • Proficient in Microsoft Excel, Word, and Outlook
  • Minimum 1 year Call center experience required
  • Minimum 1 year Healthcare experience required
  • Experience scheduling appointments for members/patients preferred
  • Medicaid experience preferred
  • Previous maternal/ pregnancy/ postpartum experience and/or knowledge preferred

  • Requires a high school diploma, GED, or equivalent or a minimum; or 3 years of related experience; or any combination of education and experience, which would provide an equivalent background. BS/BA degree in a related field preferred
  • Proficient in Microsoft Excel, Word, and Outlook
  • Minimum 2 year Call center experience
  • Minimum 2 year Healthcare experience
  • Experience scheduling appointments for members/patients preferred
  • Medicaid experience preferred
  • Previous maternal/ pregnancy/ postpartum experience and/or knowledge preferred
